State restricts Napavine rest homes

The state Department of Social and Health Services has stopped admissions to two privately operated adult family homes located in Napavine and licensed to Carlena Ashley.

The actions against Ashley Meadows and Ashley Meadows II by regulators came after inspectors found numerous violations that they judged would jeopardize the safety and well-being of residents.
Among the findings of trouble were: failure to ensure a qualified caregiver was in the home at all times, leaving residents without a qualified caregiver to meet their care and service needs; and failure to develop a system that ensured caregivers were properly trained.

The provider has the right to contest the stop placement by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home in the meantime with increased state supervision. No new admissions of residents will be allowed.
